Pathways of Care Consulting's history is supporting seniors in the community to age in place and remain as independent as possible, while supporting the next steps in their health care journey.
As Pathways of Care Consultants, we can assist our seniors and their families to the many options of care and support.
This includes navigating the system and working closely with health providers and care partners to support individuals and families. The journey of care includes Aging in place with supports, Independent and Assisted Living Options, Private Care, Memory Care, Long Term Care, and Palliative Care.
Our role in the journey of care is advocacy, informed decision-making, and facilitate collaboration with healthcare team members to ensure our seniors are heard and individual choices of care are respected and supported.
